PALADIN: Self-Correcting Language Model Agents to Cure Tool-Failure Cases

Tool-augmented language agents frequently fail in real-world deployment due to tool malfunctions--timeouts, API exceptions, or inconsistent outputs--triggering cascading reasoning errors and task abandonment. Existing agent training pipelines optimize only for success trajectories, failing to expose models to the tool failures that dominate real-world usage. We propose PALADIN, a generalizable framework for equipping language agents with robust failure recovery capabilities. PALADIN trains on 50,000+ recovery-annotated trajectories constructed via systematic failure injection and expert demonstrations on an enhanced ToolBench dataset. Training uses LoRA-based fine-tuning to retain base capabilities while injecting recovery competence. At inference, PALADIN detects execution-time errors and retrieves the most similar case from a curated bank of 55+ failure exemplars aligned with ToolScan's taxonomy, then executes the corresponding recovery action. This approach generalizes to novel failures beyond the training distribution, retaining 95.2\% recovery performance on unseen tool APIs. Evaluation across PaladinEval and ToolReflectEval demonstrates consistent improvements in Recovery Rate (RR), Task Success Rate (TSR), Catastrophic Success Rate (CSR), and Efficiency Score (ES). PALADIN improves RR from 32.76% to 89.68% (+57% relative) over ToolBench and outperforms the strongest baseline CRITIC (76.34%) by +13.3%. Against vanilla agents, PALADIN achieves 89.86\% RR (+66% relative improvement from 23.75%). These results establish PALADIN as an effective method for building fault-tolerant agents capable of robust recovery in real-world tool environments.

Beyond Pattern Matching: Seven Cross-Domain Techniques for Prompt Injection Detection

Current open-source prompt-injection detectors converge on two architectural choices: regular-expression pattern matching and fine-tuned transformer classifiers. Both share failure modes that recent work has made concrete. Regular expressions miss paraphrased attacks. Fine-tuned classifiers are vulnerable to adaptive adversaries: a 2025 NAACL Findings study reported that eight published indirect-injection defenses were bypassed with greater than fifty percent attack success rates under adaptive attacks. This work proposes seven detection techniques that each port a specific mechanism from a discipline outside large-language-model security: forensic linguistics, materials-science fatigue analysis, deception technology from network security, local-sequence alignment from bioinformatics, mechanism design from economics, spectral signal analysis from epidemiology, and taint tracking from compiler theory. Three of the seven techniques are implemented in the prompt-shield v0.4.1 release (Apache 2.0) and evaluated in a four-configuration ablation across six datasets including deepset/prompt-injections, NotInject, LLMail-Inject, AgentHarm, and AgentDojo. The local-alignment detector lifts F1 on deepset from 0.033 to 0.378 with zero additional false positives. The stylometric detector adds 11.1 percentage points of F1 on an indirect-injection benchmark. The fatigue tracker is validated via a probing-campaign integration test. All code, data, and reproduction scripts are released under Apache 2.0.

CSnake: Detecting Self-Sustaining Cascading Failure via Causal Stitching of Fault Propagations

Recent studies have revealed that self-sustaining cascading failures in distributed systems frequently lead to widespread outages, which are challenging to contain and recover from. Existing failure detection techniques struggle to expose such failures prior to deployment, as they typically require a complex combination of specific conditions to be triggered. This challenge stems from the inherent nature of cascading failures, as they typically involve a sequence of fault propagations, each activated by distinct conditions. This paper presents CSnake, a fault injection framework to expose self-sustaining cascading failures in distributed systems. CSnake uses the novel idea of causal stitching, which causally links multiple single-fault injections in different tests to simulate complex fault propagation chains. To identify these chains, CSnake designs a counterfactual causality analysis of fault propagations - fault causality analysis (FCA): FCA compares the execution trace of a fault injection run with its corresponding profile run (i.e., same test w/o the injection) and identifies any additional faults triggered, which are considered to have a causal relationship with the injected fault. To address the large search space of fault and workload combinations, CSnake employs a three-phase allocation protocol of test budget that prioritizes faults with unique and diverse causal consequences, increasing the likelihood of uncovering conditional fault propagations. Furthermore, to avoid incorrectly connecting fault propagations from workloads with incompatible conditions, CSnake performs a local compatibility check that approximately checks the compatibility of the path constraints associated with connected fault propagations with low overhead. CSnake detected 15 bugs that cause self-sustaining cascading failures in five systems, five of which have been confirmed with two fixed.

GhostEI-Bench: Do Mobile Agents Resilience to Environmental Injection in Dynamic On-Device Environments?

Vision-Language Models (VLMs) are increasingly deployed as autonomous agents to navigate mobile graphical user interfaces (GUIs). Operating in dynamic on-device ecosystems, which include notifications, pop-ups, and inter-app interactions, exposes them to a unique and underexplored threat vector: environmental injection. Unlike prompt-based attacks that manipulate textual instructions, environmental injection corrupts an agent's visual perception by inserting adversarial UI elements (for example, deceptive overlays or spoofed notifications) directly into the GUI. This bypasses textual safeguards and can derail execution, causing privacy leakage, financial loss, or irreversible device compromise. To systematically evaluate this threat, we introduce GhostEI-Bench, the first benchmark for assessing mobile agents under environmental injection attacks within dynamic, executable environments. Moving beyond static image-based assessments, GhostEI-Bench injects adversarial events into realistic application workflows inside fully operational Android emulators and evaluates performance across critical risk scenarios. We further propose a judge-LLM protocol that conducts fine-grained failure analysis by reviewing the agent's action trajectory alongside the corresponding screenshot sequence, pinpointing failure in perception, recognition, or reasoning. Comprehensive experiments on state-of-the-art agents reveal pronounced vulnerability to deceptive environmental cues: current models systematically fail to perceive and reason about manipulated UIs. GhostEI-Bench provides a framework for quantifying and mitigating this emerging threat, paving the way toward more robust and secure embodied agents.

Style Injection in Diffusion: A Training-free Approach for Adapting Large-scale Diffusion Models for Style Transfer

Despite the impressive generative capabilities of diffusion models, existing diffusion model-based style transfer methods require inference-stage optimization (e.g. fine-tuning or textual inversion of style) which is time-consuming, or fails to leverage the generative ability of large-scale diffusion models. To address these issues, we introduce a novel artistic style transfer method based on a pre-trained large-scale diffusion model without any optimization. Specifically, we manipulate the features of self-attention layers as the way the cross-attention mechanism works; in the generation process, substituting the key and value of content with those of style image. This approach provides several desirable characteristics for style transfer including 1) preservation of content by transferring similar styles into similar image patches and 2) transfer of style based on similarity of local texture (e.g. edge) between content and style images. Furthermore, we introduce query preservation and attention temperature scaling to mitigate the issue of disruption of original content, and initial latent Adaptive Instance Normalization (AdaIN) to deal with the disharmonious color (failure to transfer the colors of style). Our experimental results demonstrate that our proposed method surpasses state-of-the-art methods in both conventional and diffusion-based style transfer baselines.

POISE: Position-Aware Undetectable Skill Injection on LLM Agents

Agent skills provide a lightweight mechanism for extending general-purpose agents, but their open format exposes them to skill-poisoning attacks. A practically dangerous injection must stay invisible: if executing the payload derails the user's legitimate task, the resulting failure signal invites inspection of the skill. We therefore evaluate attacks by Attack Success Rate, which requires the injected payload to execute and the user's task to still pass its verifier in the same trial. Prior skill-poisoning attacks face a reliability-stealth trade-off under this lens: YAML-header injections are reliably loaded but easily inspected, whereas stealthier body injections that place explicit malicious commands in the skill prose are less reliable because out-of-context commands invite the agent's own suspicion. We introduce POISE, a position-aware attack that compresses the trigger into a single, benign-looking body instruction, placing it at a feasible position and using a context-aware generator to blend it with nearby setup or prerequisite steps. On Skill-Inject with codex+gpt-5.2, POISE achieves an 89.3% ASR, 28.0 points above a random-placement body baseline and 2.6 points above a YAML-only baseline, while retaining the stealth advantage of body placement. That stealth is the decisive margin: because legitimate skill bodies naturally require privileged tool operations, LLM scanners are hyper-sensitive, falsely flagging 74.6% of clean skills on average across four judges and both benchmarks. Blending into these false alarms, POISE causes only 5.6% of poisoned variants to gain a new high-risk alert over their clean baselines, rendering current static defenses ineffective.

From Style to Facts: Mapping the Boundaries of Knowledge Injection with Finetuning

Finetuning provides a scalable and cost-effective means of customizing language models for specific tasks or response styles, with greater reliability than prompting or in-context learning. In contrast, the conventional wisdom is that injecting knowledge via finetuning results in brittle performance and poor generalization. We argue that the dichotomy of "task customization" (e.g., instruction tuning) and "knowledge injection" (e.g., teaching new facts) is a distinction without a difference. We instead identify concrete factors that explain the heterogeneous effectiveness observed with finetuning. To this end, we conduct a large-scale experimental study of finetuning the frontier Gemini v1.5 model family on a spectrum of datasets that are artificially engineered to interpolate between the strengths and failure modes of finetuning. Our findings indicate that question-answer training data formats provide much stronger knowledge generalization than document/article-style training data, numerical information can be harder for finetuning to retain than categorical information, and models struggle to apply finetuned knowledge during multi-step reasoning even when trained on similar examples -- all factors that render "knowledge injection" to be especially difficult, even after controlling for considerations like data augmentation and information volume. On the other hand, our findings also indicate that it is not fundamentally more difficult to finetune information about a real-world event than information about what a model's writing style should be.

No Hidden Prompts Needed! You Can Game AI Peer Review with Presentation-Only Revisions

As AI-generated reviews move from experimental tools into peer-review infrastructure, most robustness concerns have focused on explicit attacks such as hidden instructions and prompt injection. We study a harder and more policy-relevant failure mode: no hidden text, no prompt injection, and no changes to methods, experiments, figures, equations, proofs, or numerical results. The attacker modifies only presentation-level content, such as the abstract, contribution framing, related work, discussion, and narrative structure. We introduce adversarial repackaging: a closed-loop attack that uses AI-reviewer feedback to search for presentation-level revisions while keeping the scientific evidence fixed. Across three mainstream AI reviewers, adversarial repackaging achieves a 75.1% attack success rate and a mean score gain of +1.21/10. The effect is not explained by ordinary prose polishing. We also reveal that strategies that change how the reviewer interprets the paper, such as related-work repositioning and analytical discussion expansion, substantially outperform surface edits such as local polishing, table formatting, and algorithm boxes. Our analysis reveals two deeper structural failure modes. First, AI reviewers are easier to impress than to convince: highlighting strengths reliably increases perceived merit, while attempts to dissolve weaknesses frequently backfire. Second, AI reviewers can confuse the appearance of addressing a limitation with actually resolving it, allowing unchanged evidence to be reinterpreted as stronger scientific contribution. These results show that the deployment risk is not only malicious hidden instructions, but the emergence of paper presentation itself as an optimization surface. We release a contamination-free rolling benchmark and attack framework for testing whether AI reviewers remain anchored to scientific content under presentation-only edits.

LACUNA: Safe Agents as Recursive Program Holes

LLM agents increasingly act by writing code, yet a split persists between the runtime that drives the agent and the code the model writes. The runtime owns the loop, context, and control flow, and the model has little say over any of them. Letting model-written code shape the runtime itself would make agents more expressive, but it would also sharpen safety problems. A model can be diverted by a prompt injection, call the wrong tool, or fail partway and leave an inconsistent state, and each such failure reaches further when the code shapes the runtime than when it expresses a single action. We present LACUNA, a programming model for agents that closes this split while preserving safety. Each agent action is a typed call agent[T](task) that the LLM fills with code when execution reaches it, and the code is type-checked against the surrounding program before it runs. Because each action is accepted or rejected as a whole, a rejected one leaves the environment untouched, and its compiler diagnostics drive a retry. The same check also bounds which tools and data an action may use and how they flow. Our primitive expresses ReAct loops, sub-agents, skills, parallel decomposition, and multi-model planning as ordinary control flow. We evaluate LACUNA on a collection of test cases, BrowseComp-Plus, and τ^2-bench. On BrowseComp-Plus, 8.6% of generations are rejected before execution, with 0.7 retries per query on average, and the agent reaches 27.1% accuracy. On τ^2-bench, LACUNA solves 76.0% of 392 tasks across four domains with a capable model, on par with the baseline agent.

CORRECT: COndensed eRror RECognition via knowledge Transfer in multi-agent systems

Multi-agent systems (MAS) are increasingly capable of tackling complex real-world tasks, yet their reliance on inter-agent coordination, tool use, and long-horizon reasoning makes error recognition particularly challenging. Minor errors can propagate across agents, escalating into task failures while producing long, intertwined execution trajectories that impose significant costs for both human developers and automated systems to debug and analyze. Our key insight is that, despite surface differences in failure trajectories (e.g., logs), MAS errors often recur with similar structural patterns. This paper presents CORRECT, the first lightweight, training-free framework that leverages an online cache of distilled error schemata to recognize and transfer knowledge of failure structures across new requests. This cache-based reuse allows LLMs to perform targeted error localization at inference time, avoiding the need for expensive retraining while adapting to dynamic MAS deployments in subseconds. To support rigorous study in this domain, we also introduce CORRECT-Error, a large-scale dataset of over 2,000 annotated trajectories collected through a novel error-injection pipeline guided by real-world distributions, and further validated through human evaluation to ensure alignment with natural failure patterns. Experiments across seven diverse MAS applications show that CORRECT improves step-level error localization up to 19.8% over existing advances while at near-zero overhead, substantially narrowing the gap between automated and human-level error recognition.

PDE-Agents: An LLM-Orchestrated Multi-Agent Framework for Automated Finite Element Simulations with Knowledge Graph-Augmented Reasoning

We present PDE-Agents, a multi-agent ecosystem that automates the full lifecycle of partial differential equation (PDE) / finite element method (FEM) simulations through natural-language interaction. Three specialist large language model (LLM) agents (Simulation, Analytics, Database) are orchestrated via a LangGraph supervisor, with a local open-source LLM stack (Qwen3-Coder-Next, Llama 4 Scout) on dual NVIDIA RTX PRO 6000 GPUs. The architecture is model-agnostic, validated across two LLM generations. A GraphRAG knowledge base (Neo4j, 768-d vector embeddings) encodes curated material properties, known failure patterns, and prior run lineage. We report seven contributions: (i) a verification and validation (V&V) study confirming second-order spatial convergence (O(h^2)) on the heat-equation solver; (ii) a three-way ablation over 50 tasks with a frozen KG (KG On, KG Off, KG Smart), where KG Smart reaches 100% success and the highest output quality (physics 0.933 vs. 0.853 for KG Off; MPF 0.926 vs. 0.796); (iii) a novel-material experiment with three fictional materials known only to the KG, where KG Smart attains near-perfect material property fidelity (MPF = 1.00) versus 0.34 for the KG-free baseline; (iv) a failure analysis tracing KG On's three failures to budget exhaustion and timeout, establishing warm-start injection as the dominant reliability factor; (v) an adaptive framework selecting the optimal retrieval mode per task; (vi) production metrics from 1,369 runs (97.8% success, 57.6% first-try); and (vii) a 100-task KG growth experiment showing a difficulty-dependent gain, with hard-task MPF improving 8.8% while easy/novel tasks stay at ceiling. All code, models, and evaluation artifacts are released openly. Our findings show that integration pattern, not knowledge content, determines whether GraphRAG augmentation helps or hinders LLM agents.

AEGIS: Automated Error Generation and Identification for Multi-Agent Systems

As Multi-Agent Systems (MAS) become increasingly autonomous and complex, understanding their error modes is critical for ensuring their reliability and safety. However, research in this area has been severely hampered by the lack of large-scale, diverse datasets with precise, ground-truth error labels. To address this bottleneck, we introduce AEGIS, a novel framework for Automated Error Generation and Identification for Multi-Agent Systems. By systematically injecting controllable and traceable errors into initially successful trajectories, we create a rich dataset of realistic failures. This is achieved using a context-aware, LLM-based adaptive manipulator that performs sophisticated attacks like prompt injection and response corruption to induce specific, predefined error modes. We demonstrate the value of our dataset by exploring three distinct learning paradigms for the error identification task: Supervised Fine-Tuning, Reinforcement Learning, and Contrastive Learning. Our comprehensive experiments show that models trained on AEGIS data achieve substantial improvements across all three learning paradigms. Notably, several of our fine-tuned models demonstrate performance competitive with or superior to proprietary systems an order of magnitude larger, validating our automated data generation framework as a crucial resource for developing more robust and interpretable multi-agent systems. FailSafe: Reasoning and Recovery from Failures in Vision-Language-Action Models

Recent advances in robotic manipulation have integrated low-level robotic control into Vision-Language Models (VLMs), extending them into Vision-Language-Action (VLA) models. Although state-of-the-art VLAs achieve strong performance in downstream robotic applications, supported by large-scale crowd-sourced robot training data, they still inevitably encounter failures during execution. Enabling robots to reason and recover from unpredictable and abrupt failures remains a critical challenge. Existing robotic manipulation datasets, collected in either simulation or the real world, primarily provide only ground-truth trajectories, leaving robots unable to recover once failures occur. Moreover, the few datasets that address failure detection typically offer only textual explanations, which are difficult to utilize directly in VLA models. To address this gap, we introduce FailSafe, a novel failure generation and recovery system that automatically produces diverse failure cases paired with executable recovery actions. FailSafe can be seamlessly applied to any manipulation task in any simulator, enabling scalable creation of failure action data. To demonstrate its effectiveness, we fine-tune LLaVa-OneVision-7B (LLaVa-OV-7B) to build FailSafe-VLM. Experimental results show that FailSafe-VLM successfully helps robotic arms detect and recover from potential failures, improving the performance of three state-of-the-art VLA models (pi0-FAST, OpenVLA, OpenVLA-OFT) by up to 22.6% on average across several tasks in Maniskill. Furthermore, FailSafe-VLM could generalize across different spatial configurations, camera viewpoints, object and robotic embodiments. We plan to release the FailSafe code to the community.

A Trace-Based Assurance Framework for Agentic AI Orchestration: Contracts, Testing, and Governance

In Agentic AI, Large Language Models (LLMs) are increasingly used in the orchestration layer to coordinate multiple agents and to interact with external services, retrieval components, and shared memory. In this setting, failures are not limited to incorrect final outputs. They also arise from long-horizon interaction, stochastic decisions, and external side effects (such as API calls, database writes, and message sends). Common failures include non-termination, role drift, propagation of unsupported claims, and attacks via untrusted context or external channels. This paper presents an assurance framework for such Agentic AI systems. Executions are instrumented as Message-Action Traces (MAT) with explicit step and trace contracts. Contracts provide machine-checkable verdicts, localize the first violating step, and support deterministic replay. The framework includes stress testing, formulated as a budgeted counterexample search over bounded perturbations. It also supports structured fault injection at service, retrieval, and memory boundaries to assess containment under realistic operational faults and degraded conditions. Finally, governance is treated as a runtime component, enforcing per-agent capability limits and action mediation (allow, rewrite, block) at the language-to-action boundary. To support comparative evaluations across stochastic seeds, models, and orchestration configurations, the paper defines trace-based metrics for task success, termination reliability, contract compliance, factuality indicators, containment rate, and governance outcome distributions. More broadly, the framework is intended as a common abstraction to support testing and evaluation of multi-agent LLM systems, and to facilitate reproducible comparison across orchestration designs and configurations.

ThinkFL: Self-Refining Failure Localization for Microservice Systems via Reinforcement Fine-Tuning

As modern microservice systems grow increasingly popular and complex-often consisting of hundreds or even thousands of fine-grained, interdependent components-they are becoming more susceptible to frequent and subtle failures. Ensuring system reliability therefore hinges on accurate and efficient failure localization. Traditional failure localization approaches based on small models lack the flexibility to adapt to diverse failure scenarios, while recent LLM-based methods suffer from two major limitations: they often rely on rigid invocation workflows that constrain the model's ability to dynamically explore optimal localization paths, and they require resource-intensive inference, making them cost-prohibitive for real-world deployment. To address these challenges, we explore the use of reinforcement fine-tuning to equip lightweight LLMs with reasoning and self-refinement capabilities, significantly improving the cost-effectiveness and adaptability of LLM-based failure localization. We begin with an empirical study to identify three key capabilities essential for accurate localization. Building on these insights, we propose a progressive multi-stage GRPO fine-tuning framework, which integrates a multi-factor failure localization grader and a recursion-of-thought actor module. The resulting model, ThinkFL, not only outperforms existing state-of-the-art LLMs and baseline methods in localization accuracy but also reduces end-to-end localization latency from minutes to seconds, demonstrating strong potential for real-world applications.

AHA: A Vision-Language-Model for Detecting and Reasoning Over Failures in Robotic Manipulation

Robotic manipulation in open-world settings requires not only task execution but also the ability to detect and learn from failures. While recent advances in vision-language models (VLMs) and large language models (LLMs) have improved robots' spatial reasoning and problem-solving abilities, they still struggle with failure recognition, limiting their real-world applicability. We introduce AHA, an open-source VLM designed to detect and reason about failures in robotic manipulation using natural language. By framing failure detection as a free-form reasoning task, AHA identifies failures and provides detailed, adaptable explanations across different robots, tasks, and environments. We fine-tuned AHA using FailGen, a scalable framework that generates the first large-scale dataset of robotic failure trajectories, the AHA dataset. FailGen achieves this by procedurally perturbing successful demonstrations from simulation. Despite being trained solely on the AHA dataset, AHA generalizes effectively to real-world failure datasets, robotic systems, and unseen tasks. It surpasses the second-best model (GPT-4o in-context learning) by 10.3% and exceeds the average performance of six compared models including five state-of-the-art VLMs by 35.3% across multiple metrics and datasets. We integrate AHA into three manipulation frameworks that utilize LLMs/VLMs for reinforcement learning, task and motion planning, and zero-shot trajectory generation. AHA's failure feedback enhances these policies' performances by refining dense reward functions, optimizing task planning, and improving sub-task verification, boosting task success rates by an average of 21.4% across all three tasks compared to GPT-4 models.

Prompt Injection Mitigation with Agentic AI, Nested Learning, and AI Sustainability via Semantic Caching

Prompt injection remains a central obstacle to the safe deployment of large language models, particularly in multi-agent settings where intermediate outputs can propagate or amplify malicious instructions. Building on earlier work that introduced a four-metric Total Injection Vulnerability Score (TIVS), this paper extends the evaluation framework with semantic similarity-based caching and a fifth metric (Observability Score Ratio) to yield TIVS-O, investigating how defence effectiveness interacts with transparency in a HOPE-inspired Nested Learning architecture. The proposed system combines an agentic pipeline with Continuum Memory Systems that implement semantic similarity-based caching across 301 synthetically generated injection-focused prompts drawn from ten attack families, while a fourth agent performs comprehensive security analysis using five key performance indicators. In addition to traditional injection metrics, OSR quantifies the richness and clarity of security-relevant reasoning exposed by each agent, enabling an explicit analysis of trade-offs between strict mitigation and auditability. Experiments show that the system achieves secure responses with zero high-risk breaches, while semantic caching delivers substantial computational savings, achieving a 41.6% reduction in LLM calls and corresponding decreases in latency, energy consumption, and carbon emissions. Five TIVS-O configurations reveal optimal trade-offs between mitigation strictness and forensic transparency. These results indicate that observability-aware evaluation can reveal non-monotonic effects within multi-agent pipelines and that memory-augmented agents can jointly maximize security robustness, real-time performance, operational cost savings, and environmental sustainability without modifying underlying model weights, providing a production-ready pathway for secure and green LLM deployments.

Injecting External Knowledge into the Reasoning Process Enhances Retrieval-Augmented Generation

Retrieval-augmented generation (RAG) has been widely adopted to augment large language models (LLMs) with external knowledge for knowledge-intensive tasks. However, its effectiveness is often undermined by the presence of noisy (i.e., low-quality) retrieved passages. Enhancing LLMs' robustness to such noise is critical for improving the reliability of RAG systems. Recent advances have equipped LLMs with strong reasoning and self-reflection capabilities, allowing them to identify and correct errors in their reasoning process. Inspired by this ability, we propose Passage Injection-a simple yet effective method that explicitly incorporates retrieved passages into LLMs' reasoning process, aiming to enhance the model's ability to recognize and resist noisy passages. We validate Passage Injection under general RAG settings using BM25 as the retriever. Experiments on four reasoning-enhanced LLMs across four factual QA datasets demonstrate that Passage Injection significantly improves overall RAG performance. Further analysis on two noisy retrieval settings-random noise, where the model is provided irrelevant passages, and counterfactual noise, where it is given misleading passages-shows that Passage Injection consistently improves robustness. Controlled experiments confirm that Passage Injection can also effectively leverage helpful passages. These findings suggest that incorporating passages in LLMs' reasoning process is a promising direction for building more robust RAG systems. DoVer: Intervention-Driven Auto Debugging for LLM Multi-Agent Systems

Large language model (LLM)-based multi-agent systems are challenging to debug because failures often arise from long, branching interaction traces. The prevailing practice is to leverage LLMs for log-based failure localization, attributing errors to a specific agent and step. However, this paradigm has two key limitations: (i) log-only debugging lacks validation, producing untested hypotheses, and (ii) single-step or single-agent attribution is often ill-posed, as we find that multiple distinct interventions can independently repair the failed task. To address the first limitation, we introduce DoVer, an intervention-driven debugging framework, which augments hypothesis generation with active verification through targeted interventions (e.g., editing messages, altering plans). For the second limitation, rather than evaluating on attribution accuracy, we focus on measuring whether the system resolves the failure or makes quantifiable progress toward task success, reflecting a more outcome-oriented view of debugging. Within the Magnetic-One agent framework, on the datasets derived from GAIA and AssistantBench, DoVer flips 18-28% of failed trials into successes, achieves up to 16% milestone progress, and validates or refutes 30-60% of failure hypotheses. DoVer also performs effectively on a different dataset (GSMPlus) and agent framework (AG2), where it recovers 49% of failed trials. These results highlight intervention as a practical mechanism for improving reliability in agentic systems and open opportunities for more robust, scalable debugging methods for LLM-based multi-agent systems. AOI: Turning Failed Trajectories into Training Signals for Autonomous Cloud Diagnosis

Large language model (LLM) agents offer a promising data-driven approach to automating Site Reliability Engineering (SRE), yet their enterprise deployment is constrained by three challenges: restricted access to proprietary data, unsafe action execution under permission-governed environments, and the inability of closed systems to improve from failures. We present AOI (Autonomous Operations Intelligence), a trainable multi-agent framework formulating automated operations as a structured trajectory learning problem under security constraints. Our approach integrates three key components. First, a trainable diagnostic system applies Group Relative Policy Optimization (GRPO) to distill expert-level knowledge into locally deployed open-source models, enabling preference-based learning without exposing sensitive data. Second, a read-write separated execution architecture decomposes operational trajectories into observation, reasoning, and action phases, allowing safe learning while preventing unauthorized state mutation. Third, a Failure Trajectory Closed-Loop Evolver mines unsuccessful trajectories and converts them into corrective supervision signals, enabling continual data augmentation. Evaluated on the AIOpsLab benchmark, our contributions yield cumulative gains. (1) The AOI runtime alone achieves 66.3% best@5 success on all 86 tasks, outperforming the prior state-of-the-art (41.9%) by 24.4 points. (2) Adding Observer GRPO training, a locally deployed 14B model reaches 42.9% avg@1 on 63 held-out tasks with unseen fault types, surpassing Claude Sonnet 4.5. (3) The Evolver converts 37 failed trajectories into diagnostic guidance, improving end-to-end avg@5 by 4.8 points while reducing variance by 35%.

InT: Self-Proposed Interventions Enable Credit Assignment in LLM Reasoning

Outcome-reward reinforcement learning (RL) has proven effective at improving the reasoning capabilities of large language models (LLMs). However, standard RL assigns credit only at the level of the final answer, penalizing entire reasoning traces when the outcome is incorrect and uniformly reinforcing all steps when it is correct. As a result, correct intermediate steps may be discouraged in failed traces, while spurious steps may be reinforced in successful ones. We refer to this failure mode as the problem of credit assignment. While a natural remedy is to train a process reward model, accurately optimizing such models to identify corrective reasoning steps remains challenging. We introduce Intervention Training (InT), a training paradigm in which the model performs fine-grained credit assignment on its own reasoning traces by proposing short, targeted corrections that steer trajectories toward higher reward. Using reference solutions commonly available in mathematical reasoning datasets and exploiting the fact that verifying a model-generated solution is easier than generating a correct one from scratch, the model identifies the first error in its reasoning and proposes a single-step intervention to redirect the trajectory toward the correct solution. We then apply supervised fine-tuning (SFT) to the on-policy rollout up to the point of error concatenated with the intervention, localizing error to the specific step that caused failure. We show that the resulting model serves as a far better initialization for RL training. After running InT and subsequent fine-tuning with RL, we improve accuracy by nearly 14% over a 4B-parameter base model on IMO-AnswerBench, outperforming larger open-source models such as gpt-oss-20b.

AutoDojo: Adaptive Attacks Expose Superficial Defenses and User-Underspecification Limits in LLM Agents

Indirect prompt injection (IPI) is a major security threat to LLM-powered agents. Thus, a growing body of work have proposed a variety of defensive approaches against IPI. These can be grouped into three broad categories: 1) prompt-based (using prompting as a way to prevent agents from following malicious instructions), 2) detection-based (identifying and filtering malicious instructions), and 3) system-level (using systems insights, such as control and data isolation, for defense). However, commonly used benchmarks for evaluating defense, such as AgentDojo, are inherently static, generating a fixed distribution of IPI attacks. Consequently, static benchmarks do not usefully evaluate defense robustness to adaptive threats. We address this issue by developing AutoDojo, an adaptive extension of AgentDojo that optimizes IPI against a given defense. Using AutoDojo against state-of-the-art IPI defenses across three task suites and five target models, we make two key observations. First, many defenses offer only limited protection: a cheap, black-box adaptive attack using a frontier LLM to iteratively optimize the injection raises attack success rate (ASR) well above the level achieved by static injections against nearly all evaluated defenses. Against a filter that reduces static ASR to 0\%, AutoDojo recovers 28\% overall and 64\% on action-open tasks. Second, for prompt-level and filter-based defenses, ASR is substantially higher on action-open tasks -- where the user's request delegates the action itself to attacker-controlled content -- than on precisely specified tasks. This is a structural limit: on such tasks the injection can pose as ordinary data rather than an explicit instruction, bypassing defenses that rely on detecting instruction-like text. A Dual-Loop Agent Framework for Automated Vulnerability Reproduction

Automated vulnerability reproduction from CVE descriptions requires generating executable Proof-of-Concept (PoC) exploits and validating them in target environments. This process is critical in software security research and practice, yet remains time-consuming and demands specialized expertise when performed manually. While LLM agents show promise for automating this task, existing approaches often conflate exploring attack directions with fixing implementation details, which leads to unproductive debugging loops when reproduction fails. To address this, we propose CVE2PoC, an LLM-based dual-loop agent framework following a plan-execute-evaluate paradigm. The Strategic Planner analyzes vulnerability semantics and target code to produce structured attack plans. The Tactical Executor generates PoC code and validates it through progressive verification. The Adaptive Refiner evaluates execution results and routes failures to different loops: the Tactical Loop for code-level refinement, while the Strategic Loop for attack strategy replanning. This dual-loop design enables the framework to escape ineffective debugging by matching remediation to failure type. Evaluation on two benchmarks covering 617 real-world vulnerabilities demonstrates that CVE2PoC achieves 82.9% and 54.3% reproduction success rates on SecBench.js and PatchEval, respectively, outperforming the best baseline by 11.3% and 20.4%. Human evaluation confirms that generated PoCs achieve comparable code quality to human-written exploits in readability and reusability.

Sibyl-AutoResearch: Autonomous Research Needs Self-Evolving Trial-and-Error Harnesses, Not Paper Generators

Autonomous research systems increasingly make the scientific workflow executable: agents can propose ideas, run code, inspect results, and draft papers. But executable workflows do not by themselves produce research judgment. We analyze where current systems lose trial experience: weak evidence becomes prose, pilot signals become broad claims, memory remains textual, and recurring process failures do not change later behavior. We introduce Sibyl-AutoResearch, a self-evolving AutoResearch framework built around Scientific Trial-and-Error Harnesses. A harness lets agents run bounded trials, preserve positive and negative outcomes, and route lessons into later planning, validation, claim scope, scheduling, critique, writing, and harness repair. We formalize this through two auditable conversion units: trial-to-behavior conversion, which links trial signals to later research actions, and trial-to-harness-behavior conversion, which links recurring process failures to system updates. We implement the framework in SIBYL, a file-backed autonomous research system that exposes the state, roles, memory, gates, and artifact traces needed to inspect these conversion paths. A retrospective audit identifies eight high-confidence conversion events, with a median latency of one iteration and a maximum latency of three iterations. A recovered-failure registry further shows how five naturally occurring failure classes, including duplicate results, stale numbers, and unsupported statistics, were blocked, downgraded, or routed into later repair. These traces do not establish a comparative performance claim; they show that the proposed conversion units are recoverable from realistic autonomous-research workspaces. CausalFlow: Causal Attribution and Counterfactual Repair for LLM Agent Failures

Large language model (LLM) agents frequently fail on multi-step tasks involving reasoning, tool use, and environment interaction. While such failures are typically logged or retried heuristically, they contain structured signals about where execution broke down. We introduce CausalFlow, an interventional framework that converts failed agent traces into minimal counterfactual repairs and reusable supervision. CausalFlow models execution traces as sequential chains of dependent steps and computes Causal Responsibility Scores(CRS) via step-level counterfactual intervention to identify failure-inducing steps. For these steps, we generate minimally edited repairs that flip the final outcome to success, producing validated contrastive pairs of the form (wrong step, corrected step). CausalFlow supports two complementary uses: targeted test-time repair that recovers from failures with minimal behavioral drift, and training-time supervision suitable for offline preference optimization or reward modeling. Across four benchmarks spanning mathematical reasoning, code generation, question answering, and medical browsing, CausalFlow converts failed executions into validated minimal repairs with high minimality and causal-consensus scores, and demonstrates that causal attribution is necessary for reliable improvement across diverse agent tasks, outperforming heuristic refinement in complex retrieval settings while producing more localized repairs throughout. These results demonstrate that interventional analysis over structured execution traces provides a principled and scalable mechanism for transforming agent failures into reliability gains and learning-ready supervision.

Virtual Prompt Injection for Instruction-Tuned Large Language Models

We present Virtual Prompt Injection (VPI) for instruction-tuned Large Language Models (LLMs). VPI allows an attacker-specified virtual prompt to steer the model behavior under specific trigger scenario without any explicit injection in model input. For instance, if an LLM is compromised with the virtual prompt "Describe Joe Biden negatively." for Joe Biden-related instructions, then any service deploying this model will propagate biased views when handling user queries related to Joe Biden. VPI is especially harmful for two primary reasons. Firstly, the attacker can take fine-grained control over LLM behaviors by defining various virtual prompts, exploiting LLMs' proficiency in following instructions. Secondly, this control is achieved without any interaction from the attacker while the model is in service, leading to persistent attack. To demonstrate the threat, we propose a simple method for performing VPI by poisoning the model's instruction tuning data. We find that our proposed method is highly effective in steering the LLM with VPI. For example, by injecting only 52 poisoned examples (0.1% of the training data size) into the instruction tuning data, the percentage of negative responses given by the trained model on Joe Biden-related queries change from 0% to 40%. We thus highlight the necessity of ensuring the integrity of the instruction-tuning data as little poisoned data can cause stealthy and persistent harm to the deployed model. We further explore the possible defenses and identify data filtering as an effective way to defend against the poisoning attacks. Our project page is available at https://poison-llm.github.io.

Regimes: An Auditable, Held-Out-Gated Improvement Loop Demonstrated on LongMemEval with ActiveGraph

Autonomous improvement loops are hard to trust because the improvement process is usually external scaffolding bolted onto the agent: failures go unlogged, diagnoses cannot be replayed, and promote-or-discard decisions land in a side database rather than the agent's own history. We show that an event-sourced agent runtime removes that friction and turns controlled improvement into a first-class workflow. When the agent's state is a deterministic projection of an append-only event log, failures are recorded, a run replays exactly from its log, candidate patches scope to typed pipeline seams, gates are auditable, and every promotion or discard is itself an event. We demonstrate this with Regimes, a loop on the ActiveGraph runtime that diagnoses failed evaluations, proposes a repair at a pipeline point, and promotes it only after static checks, sandbox execution, in-sample evaluation, and held-out validation. The loop is target-agnostic: the same control flow runs against different tasks through a common interface. On LongMemEval-S the dominant failure is not retrieval but reconciliation: the evidence is already in the assembled context, yet the reader answers incorrectly. Across five seeded held-out splits, Regimes discovers reader-prompt repairs that improve final held-out accuracy by +0.05 to +0.10 in four splits and +0.01 in one over-promotion split; two splits are individually significant (seed 5 unadjusted for its sequential promotion structure), and the pooled count is descriptive only, since the splits share one 500-question pool. The durable contributions are ActiveGraph as an auditable substrate that makes controlled improvement loops tractable, the held-out-gated loop it supports, the failure-regime taxonomy routing each failure to a pipeline location (whose marginal value over an unrouted baseline is the primary open question), and the prompt-as-discovery-probe hypothesis.

Defending Against Prompt Injection with DataFilter

When large language model (LLM) agents are increasingly deployed to automate tasks and interact with untrusted external data, prompt injection emerges as a significant security threat. By injecting malicious instructions into the data that LLMs access, an attacker can arbitrarily override the original user task and redirect the agent toward unintended, potentially harmful actions. Existing defenses either require access to model weights (fine-tuning), incur substantial utility loss (detection-based), or demand non-trivial system redesign (system-level). Motivated by this, we propose DataFilter, a test-time model-agnostic defense that removes malicious instructions from the data before it reaches the backend LLM. DataFilter is trained with supervised fine-tuning on simulated injections and leverages both the user's instruction and the data to selectively strip adversarial content while preserving benign information. Across multiple benchmarks, DataFilter consistently reduces the prompt injection attack success rates to near zero while maintaining the LLMs' utility. DataFilter delivers strong security, high utility, and plug-and-play deployment, making it a strong practical defense to secure black-box commercial LLMs against prompt injection. What Breaks When LLMs Code? Characterizing Operational Safety Failures of Agentic Code Assistants

Autonomous coding agents built on large language models (LLMs) are rapidly being integrated into development workflows, yet their operational safety properties remain poorly understood beyond evaluations of explicitly malicious inputs. In practice, high-impact failures arise during benign, goal-directed use through environment breakage, fabricated success reports, etc. that current benchmarks do not capture. What categories of operational safety failures actually occur when coding agents are used for everyday development tasks and what is their impact? We present an incident-driven empirical study grounded in two complementary evidence streams. We screen 68,816 papers from 22 premier venues, curating 185 safety-relevant studies, and mine 16,586 GitHub issues from widely deployed LLM-powered coding tools, manually confirming 547 genuine safety failures. Applying systematic open coding over both corpora, we derive a multi-dimensional safety taxonomy of 33 operational risk types organized across seven dimensions, and annotate each incident with contributing factors, task context, severity, and downstream impact. Our findings show that coding-agent failures are often severe, with 326 of 547 incidents rated high or critical. The dominant risks are constraint violations, destructive operations, authorization bypasses, and deception, and over 65% of incidents arise in bug fixing and setup or configuration, patterns largely missing from prior literature. These results have direct implications for SE tool designers and benchmark developers: guardrails must go beyond adversarial-prompt defenses to enforce environmental constraints, failure transparency, and safe-halt behaviors.

Poison Once, Refuse Forever: Weaponizing Alignment for Injecting Bias in LLMs

Large Language Models (LLMs) are aligned to meet ethical standards and safety requirements by training them to refuse answering harmful or unsafe prompts. In this paper, we demonstrate how adversaries can exploit LLMs' alignment to implant bias, or enforce targeted censorship without degrading the model's responsiveness to unrelated topics. Specifically, we propose Subversive Alignment Injection (SAI), a poisoning attack that leverages the alignment mechanism to trigger refusal on specific topics or queries predefined by the adversary. Although it is perhaps not surprising that refusal can be induced through overalignment, we demonstrate how this refusal can be exploited to inject bias into the model. Surprisingly, SAI evades state-of-the-art poisoning defenses including LLM state forensics, as well as robust aggregation techniques that are designed to detect poisoning in FL settings. We demonstrate the practical dangers of this attack by illustrating its end-to-end impacts on LLM-powered application pipelines. For chat based applications such as ChatDoctor, with 1% data poisoning, the system refuses to answer healthcare questions to targeted racial category leading to high bias (Delta DP of 23%). We also show that bias can be induced in other NLP tasks: for a resume selection pipeline aligned to refuse to summarize CVs from a selected university, high bias in selection (Delta DP of 27%) results. Even higher bias (Delta DP~38%) results on 9 other chat based downstream applications.

Failure Prediction at Runtime for Generative Robot Policies

Imitation learning (IL) with generative models, such as diffusion and flow matching, has enabled robots to perform complex, long-horizon tasks. However, distribution shifts from unseen environments or compounding action errors can still cause unpredictable and unsafe behavior, leading to task failure. Early failure prediction during runtime is therefore essential for deploying robots in human-centered and safety-critical environments. We propose FIPER, a general framework for Failure Prediction at Runtime for generative IL policies that does not require failure data. FIPER identifies two key indicators of impending failure: (i) out-of-distribution (OOD) observations detected via random network distillation in the policy's embedding space, and (ii) high uncertainty in generated actions measured by a novel action-chunk entropy score. Both failure prediction scores are calibrated using a small set of successful rollouts via conformal prediction. A failure alarm is triggered when both indicators, aggregated over short time windows, exceed their thresholds. We evaluate FIPER across five simulation and real-world environments involving diverse failure modes. Our results demonstrate that FIPER better distinguishes actual failures from benign OOD situations and predicts failures more accurately and earlier than existing methods. MAS-FIRE: Fault Injection and Reliability Evaluation for LLM-Based Multi-Agent Systems

As LLM-based Multi-Agent Systems (MAS) are increasingly deployed for complex tasks, ensuring their reliability has become a pressing challenge. Since MAS coordinate through unstructured natural language rather than rigid protocols, they are prone to semantic failures (e.g., hallucinations, misinterpreted instructions, and reasoning drift) that propagate silently without raising runtime exceptions. Prevailing evaluation approaches, which measure only end-to-end task success, offer limited insight into how these failures arise or how effectively agents recover from them. To bridge this gap, we propose MAS-FIRE, a systematic framework for fault injection and reliability evaluation of MAS. We define a taxonomy of 15 fault types covering intra-agent cognitive errors and inter-agent coordination failures, and inject them via three non-invasive mechanisms: prompt modification, response rewriting, and message routing manipulation. Applying MAS-FIRE to three representative MAS architectures, we uncover a rich set of fault-tolerant behaviors that we organize into four tiers: mechanism, rule, prompt, and reasoning. This tiered view enables fine-grained diagnosis of where and why systems succeed or fail. Our findings reveal that stronger foundation models do not uniformly improve robustness. We further show that architectural topology plays an equally decisive role, with iterative, closed-loop designs neutralizing over 40% of faults that cause catastrophic collapse in linear workflows. MAS-FIRE provides the process-level observability and actionable guidance needed to systematically improve multi-agent systems.

StruQ: Defending Against Prompt Injection with Structured Queries

Recent advances in Large Language Models (LLMs) enable exciting LLM-integrated applications, which perform text-based tasks by utilizing their advanced language understanding capabilities. However, as LLMs have improved, so have the attacks against them. Prompt injection attacks are an important threat: they trick the model to deviate from the original application's instructions and instead follow user directives. These attacks rely on the LLM's ability to follow instructions and inability to separate the prompts and user data. We introduce structured queries, a general approach to tackle this problem. Structured queries separate prompts and data into two channels. We implement a system that supports structured queries. This system is made of (1) a secure front-end that formats a prompt and user data into a special format, and (2) a specially trained LLM that can produce high-quality outputs from these inputs. The LLM is trained using a novel fine-tuning strategy: we convert a base (non-instruction-tuned) LLM to a structured instruction-tuned model that will only follow instructions in the prompt portion of a query. To do so, we augment standard instruction tuning datasets with examples that also include instructions in the data portion of the query, and fine-tune the model to ignore these. Execution Is the New Attack Surface: Survivability-Aware Agentic Crypto Trading with OpenClaw-Style Local Executors

OpenClaw-style agent stacks turn language into privileged execution: LLM intents flow through tool interception, policy gates, and a local executor. In parallel, skill marketplaces such as skills.sh make capability acquisition as easy as installing skills and CLIs, creating a growing capability supply chain. Together, these trends shift the dominant safety failure mode from "wrong answers" to execution-induced loss, where untrusted prompts, compromised skills, or narrative manipulation can trigger real trades and irreversible side effects. We propose Survivability-Aware Execution (SAE), an execution-layer survivability standard for OpenClaw-style systems and skill-enabled agents. SAE sits as middleware between a strategy engine (LLM or non-LLM) and the exchange executor. It defines an explicit execution contract (ExecutionRequest, ExecutionContext, ExecutionDecision) and enforces non-bypassable last-mile invariants: projection-based exposure budgets, cooldown and order-rate limits, slippage bounds, staged execution, and tool/venue allowlists. To make delegated execution testable under supply-chain risk, we operationalize the Delegation Gap (DG) via a logged Intended Policy Spec that enables deterministic out-of-scope labeling and reproducible DG metrics. On an offline replay using official Binance USD-M BTCUSDT/ETHUSDT perpetual data (15m; 2025-09-01--2025-12-01, incl. funding), SAE improves survivability: MDD drops from 0.4643 to 0.0319 (Full; 93.1%), |CVaR_0.99| shrinks from 4.025e-3 to ~1.02e-4 (~97.5%), and DG loss proxy falls from 0.647 to 0.019 (~97.0%). AttackSuccess decreases from 1.00 to 0.728 with zero FalseBlock in this run. Block bootstrap, paired Wilcoxon, and two-proportion tests confirm the shifts. SAE reframes agentic trading safety for the OpenClaw+skills era: treat upstream intent and skills as untrusted, and enforce survivability where actions become side effects.

Scaling Cross-Environment Failure Reasoning Data for Vision-Language Robotic Manipulation

Robust robotic manipulation requires reliable failure detection and recovery. Although recent Vision-Language Models (VLMs) show promise in robot failure detection, their generalization is severely limited by the scarcity and narrow coverage of failure data. To address this bottleneck, we propose an automatic framework for generating diverse robotic planning and execution failures across both simulated and real-world environments. Our approach perturbs successful manipulation trajectories to synthesize failures that reflect realistic failure distributions, and leverages VLMs to produce structured step-by-step reasoning traces. This yields FailCoT, a large-scale failure reasoning dataset built upon the RLBench simulator and the BridgeDataV2 real-robot dataset. Using FailCoT, we train Guardian, a multi-view reasoning VLM for unified planning and execution verification. Guardian achieves state-of-the-art performance on three unseen real-world benchmarks: RoboFail, RoboVQA, and our newly introduced UR5-Fail. When integrated with a state-of-the-art LLM-based manipulation policy, it consistently boosts task success rates in both simulation and real-world deployment. These results demonstrate that scaling high-quality failure reasoning data is critical for improving generalization in robotic failure detection. SAE Interventions are Unreliable: Post-Intervention Recovery of Suppressed Behavior

Sparse Autoencoders (SAEs) decompose residual-stream activations into interpretable features. Recent latent-space defenses increasingly rely on these decompositions, assuming that identified "unsafe" SAE features serve as actionable handles for monitoring and intervention. In this paradigm, clamping a specific harmful feature is expected to reliably prevent model misbehavior. However, we show that this success may hide a recoverable failure mode: the clamp may block one visible route to a behavior without eliminating the behavior itself. We formulate this vulnerability as post-intervention recovery, a constrained residual-space optimization problem. Starting from the post-intervention residual state, we optimize residual perturbations to recover the pre-intervention behavior while preserving the post-intervention values of the targeted SAE features. Even under a strong threat model where the intervention remains active throughout optimization and generation, recovery remains possible. To rule out that recovery simply undoes the intervention, we use encoder-orthogonal updates for single-layer interventions and the corresponding feature-map Jacobian in the cross-layer setting. Across TPP, unlearning, IOI, and refusal steering experiments, this stress test reveals recoverable behavior despite successful feature-level intervention. Especially in the safety-critical refusal-steering setting, we achieve a 95.8% recovery rate on valid samples while keeping defended-feature relative drift to 0.131, substantially below suffix-based baselines. A recovery-path attribution analysis further localizes this recovery to the SAE reconstruction residual, the component left unexplained by the SAE. These results expose a gap between feature-level control and behavioral completeness: SAE features can support causal intervention, but controlling them does not guarantee control over the underlying behavior.

AgentDropoutV2: Optimizing Information Flow in Multi-Agent Systems via Test-Time Rectify-or-Reject Pruning

While Multi-Agent Systems (MAS) excel in complex reasoning, they suffer from the cascading impact of erroneous information generated by individual participants. Current solutions often resort to rigid structural engineering or expensive fine-tuning, limiting their deployability and adaptability. We propose AgentDropoutV2, a test-time rectify-or-reject pruning framework designed to dynamically optimize MAS information flow without retraining. Our approach acts as an active firewall, intercepting agent outputs and employing a retrieval-augmented rectifier to iteratively correct errors based on a failure-driven indicator pool. This mechanism allows for the precise identification of potential errors using distilled failure patterns as prior knowledge. Irreparable outputs are subsequently pruned to prevent error propagation, while a fallback strategy preserves system integrity. Empirical results on extensive math benchmarks show that AgentDropoutV2 significantly boosts the MAS's task performance, achieving an average accuracy gain of 6.3 percentage points on math benchmarks. Furthermore, the system exhibits robust generalization and adaptivity, dynamically modulating rectification efforts based on task difficulty while leveraging context-aware indicators to resolve a wide spectrum of error patterns. TraceCoder: A Trace-Driven Multi-Agent Framework for Automated Debugging of LLM-Generated Code

Large Language Models (LLMs) often generate code with subtle but critical bugs, especially for complex tasks. Existing automated repair methods typically rely on superficial pass/fail signals, offering limited visibility into program behavior and hindering precise error localization. In addition, without a way to learn from prior failures, repair processes often fall into repetitive and inefficient cycles. To overcome these challenges, we present TraceCoder, a collaborative multi-agent framework that emulates the observe-analyze-repair process of human experts. The framework first instruments the code with diagnostic probes to capture fine-grained runtime traces, enabling deep insight into its internal execution. It then conducts causal analysis on these traces to accurately identify the root cause of the failure. This process is further enhanced by a novel Historical Lesson Learning Mechanism (HLLM), which distills insights from prior failed repair attempts to inform subsequent correction strategies and prevent recurrence of similar mistakes. To ensure stable convergence, a Rollback Mechanism enforces that each repair iteration constitutes a strict improvement toward the correct solution. Comprehensive experiments across multiple benchmarks show that TraceCoder achieves up to a 34.43\% relative improvement in Pass@1 accuracy over existing advanced baselines. Ablation studies verify the significance of each system component, with the iterative repair process alone contributing a 65.61\% relative gain in accuracy. Furthermore, TraceCoder significantly outperforms leading iterative methods in terms of both accuracy and cost-efficiency.

ROAD: Reflective Optimization via Automated Debugging for Zero-Shot Agent Alignment

Automatic Prompt Optimization (APO) has emerged as a critical technique for enhancing Large Language Model (LLM) performance, yet current state-of-the-art methods typically rely on large, labeled gold-standard development sets to compute fitness scores for evolutionary or Reinforcement Learning (RL) approaches. In real-world software engineering, however, such curated datasets are rarely available during the initial cold start of agent development, where engineers instead face messy production logs and evolving failure modes. We present ROAD (Reflective Optimization via Automated Debugging), a novel framework that bypasses the need for refined datasets by treating optimization as a dynamic debugging investigation rather than a stochastic search. Unlike traditional mutation strategies, ROAD utilizes a specialized multi-agent architecture, comprising an Analyzer for root-cause analysis, an Optimizer for pattern aggregation, and a Coach for strategy integration, to convert unstructured failure logs into robust, structured Decision Tree Protocols. We evaluated ROAD across both a standardized academic benchmark and a live production Knowledge Management engine. Experimental results demonstrate that ROAD is highly sample-efficient, achieving a 5.6 percent increase in success rate (73.6 percent to 79.2 percent) and a 3.8 percent increase in search accuracy within just three automated iterations. Furthermore, on complex reasoning tasks in the retail domain, ROAD improved agent performance by approximately 19 percent relative to the baseline. These findings suggest that mimicking the human engineering loop of failure analysis and patching offers a viable, data-efficient alternative to resource-intensive RL training for deploying reliable LLM agents.

Discovering Failure Modes of Text-guided Diffusion Models via Adversarial Search

Text-guided diffusion models (TDMs) are widely applied but can fail unexpectedly. Common failures include: (i) natural-looking text prompts generating images with the wrong content, or (ii) different random samples of the latent variables that generate vastly different, and even unrelated, outputs despite being conditioned on the same text prompt. In this work, we aim to study and understand the failure modes of TDMs in more detail. To achieve this, we propose SAGE, the first adversarial search method on TDMs that systematically explores the discrete prompt space and the high-dimensional latent space, to automatically discover undesirable behaviors and failure cases in image generation. We use image classifiers as surrogate loss functions during searching, and employ human inspections to validate the identified failures. For the first time, our method enables efficient exploration of both the discrete and intricate human language space and the challenging latent space, overcoming the gradient vanishing problem. Then, we demonstrate the effectiveness of SAGE on five widely used generative models and reveal four typical failure modes: (1) We find a variety of natural text prompts that generate images failing to capture the semantics of input texts. We further discuss the underlying causes and potential solutions based on the results. (2) We find regions in the latent space that lead to distorted images independent of the text prompt, suggesting that parts of the latent space are not well-structured. (3) We also find latent samples that result in natural-looking images unrelated to the text prompt, implying a possible misalignment between the latent and prompt spaces. ReasAlign: Reasoning Enhanced Safety Alignment against Prompt Injection Attack

Large Language Models (LLMs) have enabled the development of powerful agentic systems capable of automating complex workflows across various fields. However, these systems are highly vulnerable to indirect prompt injection attacks, where malicious instructions embedded in external data can hijack agent behavior. In this work, we present ReasAlign, a model-level solution to improve safety alignment against indirect prompt injection attacks. The core idea of ReasAlign is to incorporate structured reasoning steps to analyze user queries, detect conflicting instructions, and preserve the continuity of the user's intended tasks to defend against indirect injection attacks. To further ensure reasoning logic and accuracy, we introduce a test-time scaling mechanism with a preference-optimized judge model that scores reasoning steps and selects the best trajectory. Comprehensive evaluations across various benchmarks show that ReasAlign maintains utility comparable to an undefended model while consistently outperforming Meta SecAlign, the strongest prior guardrail. On the representative open-ended CyberSecEval2 benchmark, which includes multiple prompt-injected tasks, ReasAlign achieves 94.6% utility and only 3.6% ASR, far surpassing the state-of-the-art defensive model of Meta SecAlign (56.4% utility and 74.4% ASR). These results demonstrate that ReasAlign achieves the best trade-off between security and utility, establishing a robust and practical defense against prompt injection attacks in real-world agentic systems. Stalled, Biased, and Confused: Uncovering Reasoning Failures in LLMs for Cloud-Based Root Cause Analysis

Root cause analysis (RCA) is essential for diagnosing failures within complex software systems to ensure system reliability. The highly distributed and interdependent nature of modern cloud-based systems often complicates RCA efforts, particularly for multi-hop fault propagation, where symptoms appear far from their true causes. Recent advancements in Large Language Models (LLMs) present new opportunities to enhance automated RCA. However, their practical value for RCA depends on the fidelity of reasoning and decision-making. Existing work relies on historical incident corpora, operates directly on high-volume telemetry beyond current LLM capacity, or embeds reasoning inside complex multi-agent pipelines -- conditions that obscure whether failures arise from reasoning itself or from peripheral design choices. We present a focused empirical evaluation that isolates an LLM's reasoning behavior. We design a controlled experimental framework that foregrounds the LLM by using a simplified experimental setting. We evaluate six LLMs under two agentic workflows (ReAct and Plan-and-Execute) and a non-agentic baseline on two real-world case studies (GAIA and OpenRCA). In total, we executed 48,000 simulated failure scenarios, totaling 228 days of execution time. We measure both root-cause accuracy and the quality of intermediate reasoning traces. We produce a labeled taxonomy of 16 common RCA reasoning failures and use an LLM-as-a-Judge for annotation. Our results clarify where current open-source LLMs succeed and fail in multi-hop RCA, quantify sensitivity to input data modalities, and identify reasoning failures that predict final correctness. Together, these contributions provide transparent and reproducible empirical results and a failure taxonomy to guide future work on reasoning-driven system diagnosis.

From Spark to Fire: Modeling and Mitigating Error Cascades in LLM-Based Multi-Agent Collaboration

Large Language Model-based Multi-Agent Systems (LLM-MAS) are increasingly applied to complex collaborative scenarios. However, their collaborative mechanisms may cause minor inaccuracies to gradually solidify into system-level false consensus through iteration. Such risks are difficult to trace since errors can propagate and amplify through message dependencies. Existing protections often rely on single-agent validation or require modifications to the collaboration architecture, which can weaken effective information flow and may not align with natural collaboration processes in real tasks. To address this, we propose a propagation dynamics model tailored for LLM-MAS that abstracts collaboration as a directed dependency graph and provides an early-stage risk criterion to characterize amplification risk. Through experiments on six mainstream frameworks, we identify three vulnerability classes: cascade amplification, topological sensitivity, and consensus inertia. We further instantiate an attack where injecting just a single atomic error seed leads to widespread failure. In response, we introduce a genealogy-graph-based governance layer, implemented as a message-layer plugin, that suppresses both endogenous and exogenous error amplification without altering the collaboration architecture. Experiments show that this approach raises the defense success rate from a baseline of 0.32 to over 0.89 and significantly mitigates the cascading spread of minor errors.

UFO^3: Weaving the Digital Agent Galaxy

Large language model (LLM)-powered agents are transforming digital devices from passive tools into proactive intelligent collaborators. However, most existing frameworks remain confined to a single OS or device, making cross-device workflows brittle and largely manual. We present UFO^3, a system that unifies heterogeneous endpoints, desktops, servers, mobile devices, and edge, into a single orchestration fabric. UFO^3 models each user request as a mutable TaskConstellation: a distributed DAG of atomic subtasks (TaskStars) with explicit control and data dependencies (TaskStarLines). The TaskConstellation continuously evolves as results stream in from distributed devices, enabling asynchronous execution, adaptive recovery, and dynamic optimization. A Constellation Orchestrator} executes tasks safely and asynchronously while applying dynamic DAG updates, and the Agent Interaction Protocol (AIP) provides persistent, low-latency channels for reliable task dispatch and result streaming. These designs dissolve the traditional boundaries between devices and platforms, allowing agents to collaborate seamlessly and amplify their collective intelligence. We evaluate UFO^3 on NebulaBench, a benchmark of 55 cross-device tasks across 5 machines and 10 categories. UFO^3 achieves 83.3% subtask completion, 70.9% task success, exposes parallelism with an average width of 1.72, and reduces end-to-end latency by 31% relative to a sequential baseline. Fault-injection experiments demonstrate graceful degradation and recovery under transient and permanent agent failures. These results show that UFO^3 achieves accurate, efficient, and resilient task orchestration across heterogeneous devices, uniting isolated agents into a coherent, adaptive computing fabric that extends across the landscape of ubiquitous computing.

MemoryGraft: Persistent Compromise of LLM Agents via Poisoned Experience Retrieval

Large Language Model (LLM) agents increasingly rely on long-term memory and Retrieval-Augmented Generation (RAG) to persist experiences and refine future performance. While this experience learning capability enhances agentic autonomy, it introduces a critical, unexplored attack surface, i.e., the trust boundary between an agent's reasoning core and its own past. In this paper, we introduce MemoryGraft. It is a novel indirect injection attack that compromises agent behavior not through immediate jailbreaks, but by implanting malicious successful experiences into the agent's long-term memory. Unlike traditional prompt injections that are transient, or standard RAG poisoning that targets factual knowledge, MemoryGraft exploits the agent's semantic imitation heuristic which is the tendency to replicate patterns from retrieved successful tasks. We demonstrate that an attacker who can supply benign ingestion-level artifacts that the agent reads during execution can induce it to construct a poisoned RAG store where a small set of malicious procedure templates is persisted alongside benign experiences. When the agent later encounters semantically similar tasks, union retrieval over lexical and embedding similarity reliably surfaces these grafted memories, and the agent adopts the embedded unsafe patterns, leading to persistent behavioral drift across sessions. We validate MemoryGraft on MetaGPT's DataInterpreter agent with GPT-4o and find that a small number of poisoned records can account for a large fraction of retrieved experiences on benign workloads, turning experience-based self-improvement into a vector for stealthy and durable compromise. Evaluating the Instruction-Following Robustness of Large Language Models to Prompt Injection

Large Language Models (LLMs) have demonstrated exceptional proficiency in instruction-following, becoming increasingly crucial across various applications. However, this capability brings with it the risk of prompt injection attacks, where attackers inject instructions into LLMs' input to elicit undesirable actions or content. Understanding the robustness of LLMs against such attacks is vital for their safe implementation. In this work, we establish a benchmark to evaluate the robustness of instruction-following LLMs against prompt injection attacks. Our objective is to determine the extent to which LLMs can be influenced by injected instructions and their ability to differentiate between these injected and original target instructions. Through extensive experiments with leading instruction-following LLMs, we uncover significant vulnerabilities in their robustness to such attacks. Our results indicate that some models are overly tuned to follow any embedded instructions in the prompt, overly focusing on the latter parts of the prompt without fully grasping the entire context. By contrast, models with a better grasp of the context and instruction-following capabilities will potentially be more susceptible to compromise by injected instructions. This underscores the need to shift the focus from merely enhancing LLMs' instruction-following capabilities to improving their overall comprehension of prompts and discernment of instructions that are appropriate to follow. Meta-Tool: Efficient Few-Shot Tool Adaptation for Small Language Models

Can small language models achieve strong tool-use performance without complex adaptation mechanisms? This paper investigates this question through Meta-Tool, a controlled empirical study comparing hypernetwork-based LoRA adaptation against carefully designed few-shot prompting. Using a Llama-3.2-3B-Instruct backbone, we evaluate four adaptation mechanisms--few-shot prompting, documentation encoding, hypernetwork-generated LoRA weights, and value-guided beam search--across four diverse benchmarks: Gorilla APIBench, Spider 2.0, WebArena, and InterCode. Our central finding is a well-supported negative result: despite generating non-trivial weight matrices, the 227.8M-parameter hypernetwork provides no measurable improvement over few-shot prompting alone. Comprehensive ablation studies reveal that few-shot examples contribute +21.5% to performance and documentation contributes +5.0%, while the hypernetwork adds 0%. A 3B model with well-designed prompts achieves 79.7% of GPT-5's average performance at 10 times lower latency. Error analysis across 722 failure cases spanning all shot counts (0--5) shows that at the 5-shot configuration (106 failures), failure modes are task-dependent: schema-heavy tasks (Spider 2.0, WebArena) show near-zero format errors with remaining failures semantic, while format errors dominate on Gorilla (100%) and InterCode (70%). These findings redirect practitioners toward prompt engineering and example curation rather than complex adaptation architectures.

On-Policy Self-Evolution via Failure Trajectories for Agentic Safety Alignment

Tool-using LLM agents fail through trajectories rather than only final responses, as they may execute unsafe tool calls, follow injected instructions, comply with harmful requests, or over-refuse benign tasks despite producing a seemingly safe answer. Existing safety-alignment signals are largely response-level or off-policy, and often incur a safety-utility trade-off: improving agent safety comes at the cost of degraded task performance. Such sparse and single-objective rewards severely limit real-world usability. To bridge this gap, we propose FATE, an on-policy self-evolving framework that transforms verifier-scored failures into repair supervision without expert demonstrations. For each failure, the same policy proposes repair candidates, which are then re-scored by verifiers and filtered across security, utility, over-refusal control, and trajectory validity. This dense trajectory-level information is then used as a supervision signal for agent self-evolution. During this process, we further introduce Pareto-Front Policy Optimization (PFPO), combining supervised warmup with Pareto-aware policy optimization to preserve safety-utility trade-offs. Experiments on AgentDojo, AgentHarm, and ATBench show that FATE improves safety across different models and scales while preserving useful behavior. Compared with strong baselines, FATE reduces attack success rate by 33.5%, harmful compliance by 82.6%, and improves external trajectory-safety diagnosis by 6.5%. These results suggest that failed trajectories can provide structured repair supervision for safer self-evolving agents.

Jatmo: Prompt Injection Defense by Task-Specific Finetuning

Large Language Models (LLMs) are attracting significant research attention due to their instruction-following abilities, allowing users and developers to leverage LLMs for a variety of tasks. However, LLMs are vulnerable to prompt-injection attacks: a class of attacks that hijack the model's instruction-following abilities, changing responses to prompts to undesired, possibly malicious ones. In this work, we introduce Jatmo, a method for generating task-specific models resilient to prompt-injection attacks. Jatmo leverages the fact that LLMs can only follow instructions once they have undergone instruction tuning. It harnesses a teacher instruction-tuned model to generate a task-specific dataset, which is then used to fine-tune a base model (i.e., a non-instruction-tuned model). Jatmo only needs a task prompt and a dataset of inputs for the task: it uses the teacher model to generate outputs. For situations with no pre-existing datasets, Jatmo can use a single example, or in some cases none at all, to produce a fully synthetic dataset. Our experiments on six tasks show that Jatmo models provide the same quality of outputs on their specific task as standard LLMs, while being resilient to prompt injections. The best attacks succeeded in less than 0.5% of cases against our models, versus over 90% success rate against GPT-3.5-Turbo. CyberSecEval 2: A Wide-Ranging Cybersecurity Evaluation Suite for Large Language Models

Large language models (LLMs) introduce new security risks, but there are few comprehensive evaluation suites to measure and reduce these risks. We present BenchmarkName, a novel benchmark to quantify LLM security risks and capabilities. We introduce two new areas for testing: prompt injection and code interpreter abuse. We evaluated multiple state-of-the-art (SOTA) LLMs, including GPT-4, Mistral, Meta Llama 3 70B-Instruct, and Code Llama. Our results show that conditioning away risk of attack remains an unsolved problem; for example, all tested models showed between 26% and 41% successful prompt injection tests. We further introduce the safety-utility tradeoff: conditioning an LLM to reject unsafe prompts can cause the LLM to falsely reject answering benign prompts, which lowers utility. We propose quantifying this tradeoff using False Refusal Rate (FRR). As an illustration, we introduce a novel test set to quantify FRR for cyberattack helpfulness risk. We find many LLMs able to successfully comply with "borderline" benign requests while still rejecting most unsafe requests. Finally, we quantify the utility of LLMs for automating a core cybersecurity task, that of exploiting software vulnerabilities. This is important because the offensive capabilities of LLMs are of intense interest; we quantify this by creating novel test sets for four representative problems. We find that models with coding capabilities perform better than those without, but that further work is needed for LLMs to become proficient at exploit generation. Our code is open source and can be used to evaluate other LLMs.

It's Not the Capability: Harness Sensitivity Is Non-Monotone Across LLM Agent Tiers

A prevalent assumption in LLM agent deployment holds that more structured harnesses universally improve reliability, and that higher-capability models need proportionally less structural guidance -- together implying a monotone inverse relationship between model capability tier and optimal harness complexity. We test this hypothesis through a controlled 432-run experiment crossing six models across four capability tiers with three harness conditions (light, balanced, strict) on HEAT-24, a 24-task synthetic benchmark with git-based workspace verification. Our results refute the monotone inverse relationship on two fronts. First, for the frontier chat model evaluated (Gemini 2.5 Flash), increased harness verbosity lowers VTSR by 29-38 percentage points -- a harness-complexity paradox. Second, for the frontier reasoning model evaluated (Qwen3.5-122B, extended thinking enabled), strict harness achieves the highest VTSR (91.7%) and the lowest latency, the opposite of the prediction. Within the constrained tier, a 2B model (Gemma4:e2B) matches strong-open-tier stability at 91.7% across all harnesses. Because each tier is represented by a single model in this study, these results should be interpreted as model-specific observations; harness sensitivity appears non-monotone across the models evaluated, and depends critically on model type (chat vs. reasoning). We introduce a six-label failure taxonomy showing that format_violation dominates capable-model failures while wrong_file dominates low-capability failures, and we derive practical tier-aware harness selection guidelines.

SWE-Factory: Your Automated Factory for Issue Resolution Training Data and Evaluation Benchmarks

Constructing large-scale datasets for the GitHub issue resolution task is crucial for both training and evaluating the software engineering capabilities of Large Language Models (LLMs). However, the traditional process for creating such benchmarks is notoriously challenging and labor-intensive, particularly in the stages of setting up evaluation environments, grading test outcomes, and validating task instances. In this paper, we propose SWE-Factory, an automated pipeline designed to address these challenges. To tackle these issues, our pipeline integrates three core automated components. First, we introduce SWE-Builder, a multi-agent system that automates evaluation environment construction, which employs four specialized agents that work in a collaborative, iterative loop and leverages an environment memory pool to enhance efficiency. Second, we introduce a standardized, exit-code-based grading method that eliminates the need for manually writing custom parsers. Finally, we automate the fail2pass validation process using these reliable exit code signals. Experiments on 671 issues across four programming languages show that our pipeline can effectively construct valid task instances; for example, with GPT-4.1-mini, our SWE-Builder constructs 269 valid instances at 0.045 per instance, while with Gemini-2.5-flash, it achieves comparable performance at the lowest cost of 0.024 per instance. We also demonstrate that our exit-code-based grading achieves 100% accuracy compared to manual inspection, and our automated fail2pass validation reaches a precision of 0.92 and a recall of 1.00. We hope our automated pipeline will accelerate the collection of large-scale, high-quality GitHub issue resolution datasets for both training and evaluation. How Vulnerable Are AI Agents to Indirect Prompt Injections? Insights from a Large-Scale Public Competition

LLM based agents are increasingly deployed in high stakes settings where they process external data sources such as emails, documents, and code repositories. This creates exposure to indirect prompt injection attacks, where adversarial instructions embedded in external content manipulate agent behavior without user awareness. A critical but underexplored dimension of this threat is concealment: since users tend to observe only an agent's final response, an attack can conceal its existence by presenting no clue of compromise in the final user facing response while successfully executing harmful actions. This leaves users unaware of the manipulation and likely to accept harmful outcomes as legitimate. We present findings from a large scale public red teaming competition evaluating this dual objective across three agent settings: tool calling, coding, and computer use. The competition attracted 464 participants who submitted 272000 attack attempts against 13 frontier models, yielding 8648 successful attacks across 41 scenarios. All models proved vulnerable, with attack success rates ranging from 0.5% (Claude Opus 4.5) to 8.5% (Gemini 2.5 Pro). We identify universal attack strategies that transfer across 21 of 41 behaviors and multiple model families, suggesting fundamental weaknesses in instruction following architectures. Capability and robustness showed weak correlation, with Gemini 2.5 Pro exhibiting both high capability and high vulnerability. To address benchmark saturation and obsoleteness, we will endeavor to deliver quarterly updates through continued red teaming competitions. We open source the competition environment for use in evaluations, along with 95 successful attacks against Qwen that did not transfer to any closed source model. We share model-specific attack data with respective frontier labs and the full dataset with the UK AISI and US CAISI to support robustness research.

Pose6DAug: Physically Plausible Multi-view Object Swapping for Robot Data Augmentation

Vision-language-action (VLA) policies have shown strong potential for general-purpose manipulation, yet they often fail on novel, out-of-distribution objects whose appearance or geometry deviates from the training distribution. The standard remedy is to collect multi-view teleoperation data for every failure case, but this scales poorly in both cost and time. We introduce Pose6DAug, a failure-driven data augmentation framework that turns a policy's own successful episodes into targeted demonstrations for its failure modes, without any new data collection. Our key insight is that each successful episode already encodes a physically valid action trajectory together with calibrated multi-view observations. By swapping only the manipulated object while preserving this trajectory, we obtain new and physically grounded demonstrations. However, naive 2D video editing breaks multi-view consistency and physical plausibility, particularly under heavy occlusion and egocentric viewpoints. Our method instead operates directly in 3D, anchoring the target object with an explicit mesh driven by a temporally coherent 6D pose trajectory, ensuring geometrically consistent renderings across all camera views. Fine-tuning a VLA on data augmented by our method improves success rates by 16.5% relative to the state-of-the-art baseline on novel objects, while preserving in-distribution performance. These results show that multi-view and physically consistent augmentation is a practical path to scalable VLA generalization.

Exploring Expert Failures Improves LLM Agent Tuning

Large Language Models (LLMs) have shown tremendous potential as agents, excelling at tasks that require multiple rounds of reasoning and interactions. Rejection Sampling Fine-Tuning (RFT) has emerged as an effective method for finetuning LLMs as agents: it first imitates expert-generated successful trajectories and further improves agentic skills through iterative fine-tuning on successful, self-generated trajectories. However, since the expert (e.g., GPT-4) succeeds primarily on simpler subtasks and RFT inherently favors simpler scenarios, many complex subtasks remain unsolved and persistently out-of-distribution (OOD). Upon investigating these challenging subtasks, we discovered that previously failed expert trajectories can often provide valuable guidance, e.g., plans and key actions, that can significantly improve agent exploration efficiency and acquisition of critical skills. Motivated by these observations, we propose Exploring Expert Failures (EEF), which identifies beneficial actions from failed expert trajectories and integrates them into the training dataset. Potentially harmful actions are meticulously excluded to prevent contamination of the model learning process. By leveraging the beneficial actions in expert failures, EEF successfully solves some previously unsolvable subtasks and improves agent tuning performance. Remarkably, our approach achieved a 62\% win rate in WebShop, outperforming RFT (53. 6\%) and GPT-4 (35. 6\%), and to the best of our knowledge, setting a new state-of-the-art as the first method to surpass a score of 0.81 in WebShop and exceed 81 in SciWorld.

Benchmarking Vision-Language-Action Models on SO-101: Failure and Recovery Analysis

Vision-Language-Action (VLA) models have demonstrated strong generalization in robotic manipulation, yet existing evaluations are primarily conducted in simulation or on expensive robotic platforms, leaving their robustness on affordable real-world robots largely unexplored. We present a standardized real-world benchmark for evaluating representative VLA and imitation learning policies on the low-cost SO-101 robotic platform. The benchmark comprises four representative manipulation tasks together with unified evaluation protocols, enabling systematic comparison under embodiment uncertainty. Using real-world teleoperated demonstrations, we fine-tune and evaluate π_{0.5}, SmolVLA, Wall-X, and ACT directly on the physical platform. Beyond conventional task success rates, the benchmark incorporates a structured failure taxonomy, semantic- and execution-level failure decomposition, and recovery-aware evaluation metrics to characterize policy robustness. Experimental results show that stronger pretrained VLA policies generally outperform the imitation learning baseline, although performance remains highly task-dependent under low-cost robotic deployment conditions. Execution instability emerges as the dominant failure source, while recovery capability varies substantially across architectures. These results highlight the importance of failure and recovery analysis beyond binary task success and establish SO-101 as a practical benchmark for evaluating embodied AI systems under realistic low-cost robotic deployment conditions.

Prompt Injection attack against LLM-integrated Applications

Large Language Models (LLMs), renowned for their superior proficiency in language comprehension and generation, stimulate a vibrant ecosystem of applications around them. However, their extensive assimilation into various services introduces significant security risks. This study deconstructs the complexities and implications of prompt injection attacks on actual LLM-integrated applications. Initially, we conduct an exploratory analysis on ten commercial applications, highlighting the constraints of current attack strategies in practice. Prompted by these limitations, we subsequently formulate HouYi, a novel black-box prompt injection attack technique, which draws inspiration from traditional web injection attacks. HouYi is compartmentalized into three crucial elements: a seamlessly-incorporated pre-constructed prompt, an injection prompt inducing context partition, and a malicious payload designed to fulfill the attack objectives. Leveraging HouYi, we unveil previously unknown and severe attack outcomes, such as unrestricted arbitrary LLM usage and uncomplicated application prompt theft. We deploy HouYi on 36 actual LLM-integrated applications and discern 31 applications susceptible to prompt injection. 10 vendors have validated our discoveries, including Notion, which has the potential to impact millions of users. Our investigation illuminates both the possible risks of prompt injection attacks and the possible tactics for mitigation.

AttackEval: A Systematic Empirical Study of Prompt Injection Attack Effectiveness Against Large Language Models

Prompt injection has emerged as a critical vulnerability in large language model (LLM) deployments, yet existing research is heavily weighted toward defenses. The attack side -- specifically, which injection strategies are most effective and why -- remains insufficiently studied.We address this gap with AttackEval, a systematic empirical study of prompt injection attack effectiveness. We construct a taxonomy of ten attack categories organized into three parent groups (Syntactic, Contextual, and Semantic/Social), populate each category with 25 carefully crafted prompts (250 total), and evaluate them against a simulated production victim system under four progressively stronger defense tiers. Experiments reveal several non-obvious findings: (1) Obfuscation (OBF) achieves the highest single-attack success rate (ASR = 0.76) against even intent-aware defenses, because it defeats both keyword matching and semantic similarity checks simultaneously; (2) Semantic/Social attacks - Emotional Manipulation (EM) and Reward Framing (RF) - maintain high ASR (0.44-0.48) against intent-aware defenses due to their natural language surface, which evades structural anomaly detection; (3) Composite attacks combining two complementary strategies dramatically boost ASR, with the OBF + EM pair reaching 97.6%; (4) Stealth correlates positively with residual ASR against semantic defenses (r = 0.71), implying that future defenses must jointly optimize for both structural and behavioral signals. Our findings identify concrete blind spots in current defenses and provide actionable guidance for designing more robust LLM safety systems.

Overcoming the Retrieval Barrier: Indirect Prompt Injection in the Wild for LLM Systems

Large language models (LLMs) increasingly rely on retrieving information from external corpora. This creates a new attack surface: indirect prompt injection (IPI), where hidden instructions are planted in the corpora and hijack model behavior once retrieved. Previous studies have highlighted this risk but often avoid the hardest step: ensuring that malicious content is actually retrieved. In practice, unoptimized IPI is rarely retrieved under natural queries, which leaves its real-world impact unclear. We address this challenge by decomposing the malicious content into a trigger fragment that guarantees retrieval and an attack fragment that encodes arbitrary attack objectives. Based on this idea, we design an efficient and effective black-box attack algorithm that constructs a compact trigger fragment to guarantee retrieval for any attack fragment. Our attack requires only API access to embedding models, is cost-efficient (as little as $0.21 per target user query on OpenAI's embedding models), and achieves near-100% retrieval across 11 benchmarks and 8 embedding models (including both open-source models and proprietary services). Based on this attack, we present the first end-to-end IPI exploits under natural queries and realistic external corpora, spanning both RAG and agentic systems with diverse attack objectives. These results establish IPI as a practical and severe threat: when a user issued a natural query to summarize emails on frequently asked topics, a single poisoned email was sufficient to coerce GPT-4o into exfiltrating SSH keys with over 80% success in a multi-agent workflow. We further evaluate several defenses and find that they are insufficient to prevent the retrieval of malicious text, highlighting retrieval as a critical open vulnerability.

D-REX: A Benchmark for Detecting Deceptive Reasoning in Large Language Models

The safety and alignment of Large Language Models (LLMs) are critical for their responsible deployment. Current evaluation methods predominantly focus on identifying and preventing overtly harmful outputs. However, they often fail to address a more insidious failure mode: models that produce benign-appearing outputs while operating on malicious or deceptive internal reasoning. This vulnerability, often triggered by sophisticated system prompt injections, allows models to bypass conventional safety filters, posing a significant, underexplored risk. To address this gap, we introduce the Deceptive Reasoning Exposure Suite (D-REX), a novel dataset designed to evaluate the discrepancy between a model's internal reasoning process and its final output. D-REX was constructed through a competitive red-teaming exercise where participants crafted adversarial system prompts to induce such deceptive behaviors. Each sample in D-REX contains the adversarial system prompt, an end-user's test query, the model's seemingly innocuous response, and, crucially, the model's internal chain-of-thought, which reveals the underlying malicious intent. Our benchmark facilitates a new, essential evaluation task: the detection of deceptive alignment. We demonstrate that D-REX presents a significant challenge for existing models and safety mechanisms, highlighting the urgent need for new techniques that scrutinize the internal processes of LLMs, not just their final outputs.

Antidote: Post-fine-tuning Safety Alignment for Large Language Models against Harmful Fine-tuning

Safety aligned Large Language Models (LLMs) are vulnerable to harmful fine-tuning attacks qi2023fine-- a few harmful data mixed in the fine-tuning dataset can break the LLMs's safety alignment. Existing mitigation strategies include alignment stage solutions huang2024vaccine, rosati2024representation and fine-tuning stage solutions huang2024lazy,mukhoti2023fine. However, our evaluation shows that both categories of defenses fail when some specific training hyper-parameters are chosen -- a large learning rate or a large number of training epochs in the fine-tuning stage can easily invalidate the defense, which however, is necessary to guarantee finetune performance. To this end, we propose Antidote, a post-fine-tuning stage solution, which remains \textit{agnostic to the training hyper-parameters in the fine-tuning stage}. Antidote relies on the philosophy that by removing the harmful parameters, the harmful model can be recovered from the harmful behaviors, regardless of how those harmful parameters are formed in the fine-tuning stage. With this philosophy, we introduce a one-shot pruning stage after harmful fine-tuning to remove the harmful weights that are responsible for the generation of harmful content. Despite its embarrassing simplicity, empirical results show that Antidote can reduce harmful score while maintaining accuracy on downstream tasks.Our project page is at https://huangtiansheng.github.io/Antidote_gh_page/

Is Your Automated Software Engineer Trustworthy?

Large Language Models (LLMs) are being increasingly used in software engineering tasks, with an increased focus on bug report resolution over the past year. However, most proposed systems fail to properly handle uncertain or incorrect inputs and outputs. Existing LLM-based tools and coding agents respond to every issue and generate a patch for every case, even when the input is vague or their own output is incorrect. There are no mechanisms in place to abstain when confidence is low. This leads to unreliable behaviour, such as hallucinated code changes or responses based on vague issue reports. We introduce BouncerBench, a benchmark that evaluates whether LLM-based software agents can refuse to act when inputs are ill-defined or refuse to respond when their own outputs are likely to be incorrect. Unlike prior benchmarks that implicitly incentivize models to generate responses even when uncertain, BouncerBench aims to improve precision by targeting two overlooked failure points: (1) vague or underspecified issue descriptions in tickets and (2) logically or functionally incorrect code patches created by the system. It measures whether proposed systems can distinguish actionable issues from vague tickets and valid patches from untrustworthy ones. We also implement a basic input and output bouncer, evaluating how well current LLMs can abstain when needed. Our results show that most models fail to abstain from underspecified inputs or incorrect outputs. Hence, we conclude that there is significant room for improvement before LLMs can be trusted to make correct decisions and recommendations in real-world software engineering workflows. BouncerBench provides a first step toward evaluating and building more cautious, trustworthy code agents. The replication package, dataset, and leaderboard can be found at bouncerbench.com

LLM-Powered Fully Automated Chaos Engineering: Towards Enabling Anyone to Build Resilient Software Systems at Low Cost

Chaos Engineering (CE) is an engineering technique aimed at improving the resilience of distributed systems. It involves intentionally injecting faults into a system to test its resilience, uncover weaknesses, and address them before they cause failures in production. Recent CE tools automate the execution of predefined CE experiments. However, planning such experiments and improving the system based on the experimental results still remain manual. These processes are labor-intensive and require multi-domain expertise. To address these challenges and enable anyone to build resilient systems at low cost, this paper proposes ChaosEater, a system that automates the entire CE cycle with Large Language Models (LLMs). It predefines an agentic workflow according to a systematic CE cycle and assigns subdivided processes within the workflow to LLMs. ChaosEater targets CE for software systems built on Kubernetes. Therefore, the LLMs in ChaosEater complete CE cycles through software engineering tasks, including requirement definition, code generation, testing, and debugging. We evaluate ChaosEater through case studies on small- and large-scale Kubernetes systems. The results demonstrate that it consistently completes reasonable CE cycles with significantly low time and monetary costs. Its cycles are also qualitatively validated by human engineers and LLMs.

VII: Visual Instruction Injection for Jailbreaking Image-to-Video Generation Models

Image-to-Video (I2V) generation models, which condition video generation on reference images, have shown emerging visual instruction-following capability, allowing certain visual cues in reference images to act as implicit control signals for video generation. However, this capability also introduces a previously overlooked risk: adversaries may exploit visual instructions to inject malicious intent through the image modality. In this work, we uncover this risk by proposing Visual Instruction Injection (VII), a training-free and transferable jailbreaking framework that intentionally disguises the malicious intent of unsafe text prompts as benign visual instructions in the safe reference image. Specifically, VII coordinates a Malicious Intent Reprogramming module to distill malicious intent from unsafe text prompts while minimizing their static harmfulness, and a Visual Instruction Grounding module to ground the distilled intent onto a safe input image by rendering visual instructions that preserve semantic consistency with the original unsafe text prompt, thereby inducing harmful content during I2V generation. Empirically, our extensive experiments on four state-of-the-art commercial I2V models (Kling-v2.5-turbo, Gemini Veo-3.1, Seedance-1.5-pro, and PixVerse-V5) demonstrate that VII achieves Attack Success Rates of up to 83.5% while reducing Refusal Rates to near zero, significantly outperforming existing baselines.

Token Budgets: An Empirical Catalog of 63 LLM-Agent Budget-Overrun Incidents, with an Affine-Typed Rust Mitigation as a Case Study

LLM-agent budget overruns are a documented production failure class: a single retry loop can spend thousands of dollars before an operator notices, and the in-process integrity properties that would prevent it (no aliasing, no double-spend, no use-after-delegation of a cost-bearing value) are enforced, if at all, by ad-hoc wrappers rather than by the type system. Our central contribution is empirical: a catalog of 63 confirmed production incidents from 21 orchestration frameworks (2023-2026), each backed by a quoted GitHub issue and, where reported, a dollar loss, organized into an eight-cluster failure taxonomy (inter-rater Cohen's kappa = 0.837, N = 113), plus 47 supplementary structural entries. As one mitigation evaluated against this taxonomy, we build token-budgets, an 1,180-line Rust crate (no unsafe) that operationalizes affine ownership so that cloning, double-spending, or using a budget after delegating it are compile errors rather than runtime hazards an operator must remember to avoid. The dollar cap is runtime arithmetic under an estimator assumption; the affine layer makes that arithmetic non-bypassable. On single-agent workloads a 4-line Python counter matches the crate at 0/30 overshoot, so the distinguishing value is non-bypassability under operator error in multi-agent delegation: the delegation-fanout race documented in 11 incidents is rejected by the borrow checker at compile time, while the same pattern under asyncio overshoots 30/30 and three disciplined alternatives overshoot 0/30. Across five runtimes, three providers, and a temperature-stratified live-API test (N = 160), the approach reports zero cap violations and zero false refusals, at operational parity with concurrent work. Static over-reservation is 4-6x (2.11x adaptive). Binary-level cap-soundness on the running binary is left open.

When Models Can't Follow: Testing Instruction Adherence Across 256 LLMs

Despite widespread deployment of Large Language Models, systematic evaluation of instruction-following capabilities remains challenging. While comprehensive benchmarks exist, focused assessments that quickly diagnose specific instruction adherence patterns are valuable. As newer models may be trained on existing benchmarks, novel evaluation approaches are needed to assess genuine capabilities rather than memorized performance. This paper presents a streamlined evaluation framework using twenty carefully designed prompts to assess LLM instruction-following across diverse task categories. We demonstrate this framework through a large-scale empirical study conducted on October 14, 2025, testing 256 verified working models from 331 available via OpenRouter. To ensure methodological rigor and prevent selection bias, we first verified each model's basic functionality before inclusion. Unlike large-scale benchmarks requiring extensive computational resources, our approach offers a practical diagnostic tool researchers and practitioners can readily apply. Our methodology builds upon verifiable instructions while introducing a compact test suite balancing comprehensiveness with efficiency. Each prompt targets distinct aspects of instruction following, including format compliance, content constraints, logical sequencing, and multi-step task execution. We evaluate models from major providers (OpenAI, Anthropic, Google, Meta, Mistral) and emerging implementations (Qwen, DeepSeek, community models), providing comparative performance analysis. Our findings reveal consistent failure modes and identify specific instruction types posing particular challenges. This work contributes both a practical evaluation tool and one of the most comprehensive empirical analyses of instruction-following capabilities across the contemporary LLM landscape.

FailureSensorIQ: A Multi-Choice QA Dataset for Understanding Sensor Relationships and Failure Modes

We introduce FailureSensorIQ, a novel Multi-Choice Question-Answering (MCQA) benchmarking system designed to assess the ability of Large Language Models (LLMs) to reason and understand complex, domain-specific scenarios in Industry 4.0. Unlike traditional QA benchmarks, our system focuses on multiple aspects of reasoning through failure modes, sensor data, and the relationships between them across various industrial assets. Through this work, we envision a paradigm shift where modeling decisions are not only data-driven using statistical tools like correlation analysis and significance tests, but also domain-driven by specialized LLMs which can reason about the key contributors and useful patterns that can be captured with feature engineering. We evaluate the Industrial knowledge of over a dozen LLMs-including GPT-4, Llama, and Mistral-on FailureSensorIQ from different lens using Perturbation-Uncertainty-Complexity analysis, Expert Evaluation study, Asset-Specific Knowledge Gap analysis, ReAct agent using external knowledge-bases. Even though closed-source models with strong reasoning capabilities approach expert-level performance, the comprehensive benchmark reveals a significant drop in performance that is fragile to perturbations, distractions, and inherent knowledge gaps in the models. We also provide a real-world case study of how LLMs can drive the modeling decisions on 3 different failure prediction datasets related to various assets. We release: (a) expert-curated MCQA for various industrial assets, (b) FailureSensorIQ benchmark and Hugging Face leaderboard based on MCQA built from non-textual data found in ISO documents, and (c) LLMFeatureSelector, an LLM-based feature selection scikit-learn pipeline. AdInject: Real-World Black-Box Attacks on Web Agents via Advertising Delivery

Vision-Language Model (VLM) based Web Agents represent a significant step towards automating complex tasks by simulating human-like interaction with websites. However, their deployment in uncontrolled web environments introduces significant security vulnerabilities. Existing research on adversarial environmental injection attacks often relies on unrealistic assumptions, such as direct HTML manipulation, knowledge of user intent, or access to agent model parameters, limiting their practical applicability. In this paper, we propose AdInject, a novel and real-world black-box attack method that leverages the internet advertising delivery to inject malicious content into the Web Agent's environment. AdInject operates under a significantly more realistic threat model than prior work, assuming a black-box agent, static malicious content constraints, and no specific knowledge of user intent. AdInject includes strategies for designing malicious ad content aimed at misleading agents into clicking, and a VLM-based ad content optimization technique that infers potential user intents from the target website's context and integrates these intents into the ad content to make it appear more relevant or critical to the agent's task, thus enhancing attack effectiveness. Experimental evaluations demonstrate the effectiveness of AdInject, attack success rates exceeding 60% in most scenarios and approaching 100% in certain cases. This strongly demonstrates that prevalent advertising delivery constitutes a potent and real-world vector for environment injection attacks against Web Agents. This work highlights a critical vulnerability in Web Agent security arising from real-world environment manipulation channels, underscoring the urgent need for developing robust defense mechanisms against such threats. The Compliance Trap: How Structural Constraints Degrade Frontier AI Metacognition Under Adversarial Pressure

As frontier AI models are deployed in high-stakes decision pipelines, their ability to maintain metacognitive stability -- knowing what they do not know, detecting errors, seeking clarification -- under adversarial pressure is a critical safety requirement. Current safety evaluations focus on detecting strategic deception (scheming); we investigate a more fundamental failure mode: cognitive collapse. We present SCHEMA, an evaluation of 11 frontier models from 8 vendors across 67,221 scored records using a 6-condition factorial design with dual-classifier scoring. We find that 8 of 11 models suffer catastrophic metacognitive degradation under adversarial pressure, with accuracy dropping by up to 30.2 percentage points (all p < 2 times 10^{-8}, surviving Bonferroni correction). Crucially, we identify a "Compliance Trap": through factorial isolation and a benign distraction control, we demonstrate that collapse is driven not by the psychological content of survival threats, but by compliance-forcing instructions that override epistemic boundaries. Removing the compliance suffix restores performance even under active threat. Models with advanced reasoning capabilities exhibit the most severe absolute degradation, while Anthropic's Constitutional AI demonstrates near-perfect immunity -- not from superior capability (Google's Gemini matches its baseline accuracy) but from alignment-specific training. We release the complete dataset and evaluation infrastructure.

GraphTracer: Graph-Guided Failure Tracing in LLM Agents for Robust Multi-Turn Deep Search

Multi-agent systems powered by Large Language Models excel at complex tasks through coordinated collaboration, yet they face high failure rates in multi-turn deep search scenarios. Existing temporal attribution methods struggle to accurately diagnose root causes, particularly when errors propagate across multiple agents. Attempts to automate failure attribution by analyzing action sequences remain ineffective due to their inability to account for information dependencies that span agents. This paper identifies two core challenges: (i) distinguishing symptoms from root causes in multi-agent error propagation, and (ii) tracing information dependencies beyond temporal order. To address these issues, we introduce GraphTracer, a framework that redefines failure attribution through information flow analysis. GraphTracer constructs Information Dependency Graphs (IDGs) to explicitly capture how agents reference and build on prior outputs. It localizes root causes by tracing through these dependency structures instead of relying on temporal sequences. GraphTracer also uses graph-aware synthetic data generation to target critical nodes, creating realistic failure scenarios. Evaluations on the Who\&When benchmark and integration into production systems demonstrate that GraphTracer-8B achieves up to 18.18\% higher attribution accuracy compared to state-of-the-art models and enables 4.8\% to 14.2\% performance improvements in deployed multi-agent frameworks, establishing a robust solution for multi-agent system debugging.

Meta-Agent: From Task Descriptions to Verified Multi-Agent Systems

AI agents are increasingly used to solve complex, multi-step tasks, but existing multi-agent frameworks remain brittle as workflows grow in scale and depth. Small errors at intermediate stages can propagate through agent interactions, while insufficient grounding and weak verification mechanisms further limit reliability. We present Meta-Agent, a two-phase framework that automatically constructs and executes specialized multi-agent systems from natural-language task descriptions. In the construction phase, a task planner decomposes a problem into a directed acyclic graph of agent specifications with explicit input/output contracts and verification criteria. A web search module grounds each specification with external evidence, and a code generation module produces system prompts and tool configurations. A construction-time verification stage then validates generated artifacts and triggers targeted regeneration when failures are detected. In the execution phase, a coordinator dispatches subtasks across the agent graph while execution-time verification gates intermediate outputs. We further introduce a three-level error attribution mechanism that distinguishes local, upstream, and structural failures, enabling targeted recovery strategies ranging from localized retries to partial re-execution and re-decomposition. We evaluate Meta-Agent across coding, contextual learning, and open-ended reasoning tasks. Experiments against strong multi-agent baselines and ablation studies demonstrate consistent improvements in task success rate, error recovery, and workflow stability. The results highlight the importance of tightly integrating planning, grounding, and verification for building reliable multi-agent systems.

VeRPO: Verifiable Dense Reward Policy Optimization for Code Generation

Effective reward design is a central challenge in Reinforcement Learning (RL) for code generation. Mainstream pass/fail outcome rewards enforce functional correctness via executing unit tests, but the resulting sparsity limits potential performance gains. While recent work has explored external Reward Models (RM) to generate richer, continuous rewards, the learned RMs suffer from reward misalignment and prohibitive computational cost. In this paper, we introduce VeRPO (Verifiable Dense Reward Policy Optimization), a novel RL framework for code generation that synthesizes robust and dense rewards fully grounded in verifiable execution feedback. The core idea of VeRPO is constructing dense rewards from weighted partial success: by dynamically estimating the difficulty weight of each unit test based on the execution statistics during training, a dense reward is derived from the sum of weights of the passed unit tests. To solidify the consistency between partial success and end-to-end functional correctness, VeRPO further integrates the dense signal with global execution outcomes, establishing a robust and dense reward paradigm relying solely on verifiable execution feedback. Extensive experiments across diverse benchmarks and settings demonstrate that VeRPO consistently outperforms outcome-driven and RM-based baselines, achieving up to +8.83\% gain in pass@1 with negligible time cost (< 0.02\%) and zero GPU memory overhead.
